    ApprovalQueueDecisionHandler
    The queue gate (SPEC item 6, D-03): every run path goes through Queue.QueueDecisionHandler, so approval-required jobs are controlled at one point.

    Cause carried by every build that runs on behalf of an approved run request (SPEC item 5): links the build to the request id, the requester and the approver.

    Invisible marker action that authorizes exactly one queue submission for an approved run request (D-23).
